Athlete Clearance Forms
In order to participate in the athletic program for the 2013-2014 school year the following forms, along with a $125 athletic fee must be submitted to the Athletic Office by July 8 for girls volleyball, July 15 for cross country, August 9 for football, August 12 for pep squad, and August 19 for girls tennis and girls golf. Winter and spring sports will be due during the school year prior to their tryout or practice. ATHLETES WILL NOT BE ALLOWED TO TRY OUT, PRACTICE OR PARTICIPATE UNTIL ALL FORMS AND FEE ARE RECEIVED.
(1) Physical Form – physicals will be given on campus in the respective locker rooms on:
Tuesday July 2 9:00-10:30 Girls Locker Room
Tuesday July 2 10:30-12:00 Boys Locker Room
Tuesday July 9 9:00-10:30 Girls Locker Room
Tuesday July 9 10:30-12:00 Boys Locker Room
Physicals will not be given on campus during the school year so we encourage athletes playing a winter and/or spring sport to take advantage of the above physical times. The cost of the physical is $20.00 (cash only) and due at the time of the physical. You may visit your own doctor but you must bring this form to your doctor.
(2) Emergency/Consent/Proof of Insurance Form - Insurance coverage can be provided by the parent's insurance carrier or through a basic insurance policy offered at VCHS on an optional basis to all students (VCHS policy requires special coverage for football).

(4) Athletic Fee - $125. Each athlete is required to pay a yearly fee (not per sport). You may request a refund of the athletic fee should your child not be selected for a team and they do not play any other sport during the year.
If you wish to take your child home from a game in your own car, please have this form to the head coach 24 hours prior to the event so transportation can make appropriate modifications. Please fill out a new form for each event. One form is not sufficient for the whole season. If your child plans to ride the bus home from each event, this form does not need to be filled out.
